WISe Program

Wraparound with Intensive Services (WISe) Program - For Medicaid Families

Wraparound with Intensive Services (WISe) uses a team approach to provide intensive mental health services to Apple Health/Medicaid-eligible youth with complex needs. It is designed to provide individualized, culturally aware services that keep youth safe in their own homes and communities, reducing the need for hospitalizations and inpatient treatment.

WISe APPROACH

The program honors the family’s voice and choice, identifies strengths, builds natural supports, and fosters self-sufficiency. The WISe team collaborates with families, crossed-system partners, and natural supports to create positive change. Teams provide care coordination, therapy, peer services, psychiatric services, and 24/7 crisis response.

TEAM-BASED

Each enrolled family is assigned a team of YES staff to give them the best chance at success.

  • Care coordinator who organizes and manages the collaboration between the team and family and provides community-based resources.
  • Family support partner who is a parent with lived experience that can offer the parent/caregiver guidance.
  • Youth peer support partner who supports the youth’s engagement and advocates for their voice.
  • Master’s-level therapist who provides treatment that builds on the youth’s and family’s strengths and culture.
  • Psychiatrist (if needed) who provides help with medication management.

ELIGIBLE CLIENTS

WISe Services are available for youth ages 6-20 and their families living in Bellevue, Redmond, or Kirkland. Youth clients must have Medicaid (or be Medicaid eligible) to qualify for this program.

The program is designed for youth:

  • Frequently using crisis lines or emergency rooms due to mental health concerns.
  • Experiencing hard to understand behavior (such as running away or frequent arrests that are due to mental health) that are challenging to you, other caregivers, or treatment provider, and traditional services alone are not helping.
  • Displaying an elevated risk of harm to themselves or others.
  • In need of a more intensive and individualized approach to treatment.
  • In need of a more flexible and engaging approach.
  • Involved in multiple systems (like mental health, CPS, Juvenile Justice, developmental disability services, and/or substance use disorder treatment) and the other agencies that are struggling to support your child together.
  • In special education and/or has a 504 Plan, with multiple school suspensions for mental health and/or behavioral issues.
